normal align=left style='text-align:left;text-indent:28.0pt;mso-char-indent-count:2.0;line-height:28.0pt;mso-line-height-rule:exactly'>3、拟派的总监理工程师必须具有建筑工程专业注册监理工程师执业资格且在该企业注册，须提供2019年6、7、8月份的社会劳动保险证明（须有社保业务部门的印章），社会劳动保险证明所属单位必须与投标人名称一致，且证明出具时间以公告时间段为准。

normal align=left style='text-align:left;text-indent:28.0pt;mso-char-indent-count:2.0;line-height:26.0pt;mso-line-height-rule:exactly'>7、投标人拟派总监理工程师须携带本人身份证、营业执照、资质证书、基本账户开户许可证或银行出具的企业账户证明材料、监理工程师注册执业证书等原件及复印件（加盖公章）一份联系代理机构进行核查，原件核查后并退回。必须在2019年10月10日至2019年  10月14日（上午9：30至11：30，下午14：30至16：00，北京时间）联系招标代理机构签署诚信投标及项目管理班子驻工地现场承诺书，过时不候，投标登记时提供承诺书原件（承诺书格式见本公告附件二）
